The case report describes a patient presented with chest pain . ECG reveals a STEMI over V1 through V3 representing a anteroseptal MI where the culprit vessel expected is proximal LAD but paradoxically CAG revealed proximal RCA occlusion. This is possibly because of RV infraction.
